About this template
Here is a modern way to record training that has been completed: the Plum Chart Optometry CPD Certificate, a template that suits clinics, therapy practices and allied health educators. Look closely and you will notice chart lines and a bold band of colour, set off by Atkinson Hyperlegible lettering in plum.
Suggested wording
Copy one of these lines into the certificate, then replace the words in brackets.
Awarded to [Name] for completing Optometry CPD in line with [Standard or policy].
[Name] has successfully completed Optometry CPD and is competent in the procedures covered. Valid until [Expiry date].
This certifies that [Name] has completed Optometry CPD ([4] CPD hours) on [Date], delivered by [Trainer].
How to customise it
Click any line to change it: the recipient’s name, the wording, the date, visual acuity, CPD points and competency and the signatures. Fonts can be swapped too; the defaults are Atkinson Hyperlegible and Public Sans. Colours are yours to change: try Clinic Teal, or match your own brand colours. Choose A4, US Letter or A3, in landscape or portrait, and the layout rearranges itself to fit. You can also upload a logo or a photo and place it anywhere on the page.
Printing tips
Bright white matte paper of 160–200 gsm keeps the colours crisp. Print one copy on plain paper first to check the spelling and the margins. Standard A4 and Letter frames fit it without trimming.
