About this template
Here is a clean way to record training that has been completed: the Forest Green Sunburst Phlebotomy Training Certificate, a template that suits clinics, therapy practices and allied health educators. Look closely and you will notice a sunburst and a badge, set off by Mukta lettering in forest green.
Suggested wording
Copy one of these lines into the certificate, then replace the words in brackets.
[Name] has completed the Phlebotomy Training. Please keep this certificate with your training records.
This certifies that [Name] has completed the Phlebotomy Training ([4] CPD hours) on [Date], delivered by [Trainer].
This is to confirm that [Name] attended the Phlebotomy Training on [Date] and passed the practical assessment.
How to customise it
Every word is editable, from the title to the signature line, including the order of draw, supervised draws and assessment lines. Fonts can be swapped too; the defaults are Mukta and Source Sans 3. Choose A4, US Letter or A3, in landscape or portrait, and the layout rearranges itself to fit. The design comes with three colour schemes (Clinical Red, Teal Ward and Graphite & Berry), and every single colour can be changed. You can also upload a logo or a photo and place it anywhere on the page.
Printing tips
Bright white matte paper of 160–200 gsm keeps the colours crisp. In the print dialog choose “Actual size” rather than “Fit”, so the border isn’t cropped. It fits a standard certificate frame or a document folder.
