About this template
Whether it is for one person or a whole group, the Chocolate Seal CME Attendance Certificate gives hospitals, clinics and care homes a clean way to record training that has been completed. With a round seal and Sora headings, it has a clean feel, while the Oxblood & Stone palette keeps the colours fresh. Change a few words and it doubles as a certificate of participation.
Suggested wording
Copy one of these lines into the certificate, then replace the words in brackets.
Presented to [Name] on completing CME Attendance Certificate with [Organisation]. Thank you for keeping everyone safe.
This is to confirm that [Name] attended CME Attendance Certificate on [Date] and passed the practical assessment.
Awarded to [Name] for completing CME Attendance Certificate in line with [Standard or policy].
How to customise it
You can rewrite the whole certificate; the venue, format and activity ID fields are already in place, and you can delete the ones you don’t need. Fonts can be swapped too; the defaults are Sora and Literata. Colours are yours to change: try Clinical Navy, or match your own brand colours. Need portrait instead of landscape, or US Letter instead of A4? Change it in one click. Add your logo, a crest or a photo and drag it into place.
Printing tips
Use a smooth white paper rather than parchment, so the clean lines stay sharp. Print one copy on plain paper first to check the spelling and the margins. It fits a standard certificate frame or a document folder.
